We are seeking a PRN Home Health LPN for the Indianapolis area. Job duties of this rewarding position include:
- Providing direct patient care to home health patients, implementing the plan of care initiated by the RN
- Providing accurate and timely documentation consistent with the plan of care
- Assessing and providing patient and family/caregiver education and information pertinent to diagnosis and plan of care
- Assisting the RN or physician in performing specialized procedures and duties
- Participating in coordination of home health services, appropriately reporting the need for other disciplines
Ideal candidates should have the following qualifications:
- Graduate of an accredited practical nurse or vocational nursing program
- Three years nursing experience, home health or med/surg preferred
- Currently licensed as an LPN in Indiana
- Must be a licensed driver with an automobile in good working order
